Home Care Packages
If you want to stay in your own home as you get older, but need some help with things like cleaning and preparing meals, or with transport so you can go shopping or attend appointments, a Home Care Package may help you.

Advance Care Directive is a legal document where a person is able to write down his or her instructions, wishes and preference for the future health care, accomodation and personal matters, and/or appoint one or more substitute decision makers who are people chosen to made decisions on his or her behalf.

Seniors Card
To be eligible for a Seniors Card you must meet the following criteria: 60 years or older, a permanent South Australian resident, not working more than 20 hours per week in paid employment (part-time and casual employees may average their hours over a 12 month period).

Wills
A Will is a legal document that allows you to make clear what you want to happen to your estate once you are gone. By having a Will you are able to leave instructions for the distribution of your estate and you can name someone as the executor of your Will to carry out these instructions on your behalf.

Financial Advice
Financial planners are often referred to in a variety of ways: as financial planners and (where they are linked to an Australian Financial Services Licensee, such as Charter Financial Planning) as authorised representatives.

Superannuation
Superannuation is a way to save for your retirement. The money comes from contributions made into your super fund by your employer and, ideally, topped up by your own money. Sometimes the government will add to it through co-contributions and the low income super contribution.

Transition to Retirement
Planning for retirement involves not just finance and superannuation. It also includes your physical, social and emotional well-being, as well as keeping your mind active and staying motivated.

Residential Care
Residential Aged Care Facilities provide various levels of supportive care and are administered and operated under the Aged Care Act 1997. An Aged Care Assessment is required to gain entry into these types of facilities. All aged care facilities undergo an accrediation process to ensure quality of care for the older person.

Low Income Housing
Low Income Housing provides long term housing, security and affordable rent (approximatley 25% of Income) for people on low incomes. Community Housing organisations are non-government organisations operating under the Communtiy Housing Providers (National Law) (South Australia) Act 2013.

Retirement Villages
Retirement villages are complexes of residential units or a number of separate complexes of residential units on common land. These units are intended for residential use under a retirement village scheme.

Independent Living Units
Independent Living Units (referred to as villas or ILU's) are standard units which are designed for the person to live independently and enjoy the security of a gated community with peers close by.

Aged Care Complaints
The Aged Care Complaints Commissioner provides a free service for anyone to raise their concerns about the quality of care or services being delivered to people receiving aged care services funded by the Australian Government.

Aged Rights Advocacy
Aged Rights Advocacy service is an independent, rights based organisation, providing advocacy support. It is an independent, rights based organisation which aims to promote and protect the rights and well-being of older people.

Personal Alarms
A personal alert system is a device that enables people to call for help in an emergency if they are unable to access or use a telephone.

Dementia Safe Return
The Safe Return is a joint initiative between Alzheimer’s Australia South Australia, and the South Australia Police to help ensure the safety of people at risk of becoming lost. The program enables the rapid identification and return home of people who become lost by using a unique ID code engraved on a secure bracelet. The details of the person at risk of becoming lost are accessible only by you and the SA Police.

MedicAlert Bracelets
Medical ID provides for a quick regognition of your medical conditions, allergies, medications or treatmnet wishes this leads to faster and more effective medical treatment.Medical ID braclets reduce treatment errors which may result from not having a paitents health record during an emergency situation or upon hospital admission.

Guardianship
A person who is unable to make certain decisions about their personal health and living circumstances due to mental incapacity has their rights and interests protected by the law. SACAT can appoint a guardian under a guardianship order to make accommodation, health and lifestyle decisions to support a person who has mental incapacity.

The Australian Government has lauched the Carer Gateway which is is a national online and phone service that provides practical information and resources to support carers. The interactive service finder helps carers connect to local support services.

The Department of Veterans Affairs provides a variety of in-home and community support programmes that focus on assisting veteran's to continue living independently.

Government Funded Services
The Australian Government has launched My Aged Care which is is a national online and phone service that provides practical information and resources to support older people. The interactive service finder helps carers connect to local support services. This is the new "Gateway" to Aged care services.

Mental Health and Private Services
The focus of Older Persons Mental Health Service (OPMHS) in South Australia is to support older people to maximise their mental health. This is achieved through working with the community, ageing and primary health care sectors to prevent or reduce the incidence and/or escalation of mental health issues.

Private Services
Private Service - An aged care or related service delivered by a Service Provider who is not directly subsidised for that service under any applicable Commonwealth Aged Care Programmes. Access to these services and any fees that may be applied should be discussed directly with the provider.

Respite Services
If you’re caring for someone, you might find it hard to take a break. But it is important that you do – both for you and for the person you care for. It might be for just a short time, like a few hours or overnight, or it can be for longer periods, like a few weeks.

Health Services
The Department of Veterans Affairs funds a range of health services that are clinically required to treat a veteran's particular health conditions.

Equipment Services
The Rehabilitation Appliance Program helps veterans to be independent and self-reliant in their own homes. Health care assessments and the provision of aids and appliances help to minimise the impact of disabilities, enhance quality of life and maximise independence. RAP provides equipment according to each person's assessed clinical need as part of the overall management of their health care.

Veterans' Home Care
Veterans' Home Care is a Department of Veterans Affairs program designed to assist veterans who need a small amount of practical help to continue living independently in their own home. Services include Domestic Assistance, Personal Care, Respite Care, and safety-related Home and Garden Maintenance. Call a VHC Assessment Agency on 1300 550 450 to discuss your eligibility.

Community Nursing
Community nursing provides clinical nursing and/or personal care services to eligible veterans in their own home. Community nursing services can assist with medication, wound care, hygiene and help with showering or dressing.

Community and Home Support
The Commonwealth Home Support Programme is an entry level home help programme funded by the Australian Government. If you are an older people who can mostly – but not completely – live and cope on your own, and don’t yet need higher levels of support at home, you may be eligible for this support.

Carer Relationships
Caring for a friend or family member can be physically and emotionally tiring. In-home respite can help support the person you care for in their home and can provide you an opportunity to have a break.

Assistance with Care and Housing
Some people are at risk of becoming homeless as they get older. This might be because they are renting or have insecure housing and face challenges finding a place to live that they can call a home. The Commonwealth Home Support Programme may be able to help.

Residential Care
Residential aged care is for older people, who for a variety of reasons, can no longer live at home. Those reasons can include illness, disability, bereavement, an emergency, the changing needs of their carer, family or friends or because it is no longer possible to manage at home
